It’s no secret that Blake Lively has captivated audiences with her stunning beauty and sophisticated fashion sense since her ascent to stardom on Gossip Girl in the 2000s. In a comprehensive YouTube video, plastic surgeon Dr. Gary Linkov analyzed Lively’s evolution over the years, offering his insights into the subtle cosmetic enhancements she may have undergone, which he estimates could have totaled around $125,000.
At the beginning of the video, Linkov said, regarding the 37-year-old It Ends with Us actress: “Blake Lively has long been admired for her looks, but as time has passed many have wondered if her evolving look is purely natural.” He added: “Today, we’ll explore her journey, examining whether these changes are a result of natural aging, makeup techniques or perhaps even plastic surgery.”

The doctor subsequently contrasted a picture of Lively taken when she was 19 in 2006 with one from her high school years. "When we look at her nose, we do see some nasal tip bulbosity and there's a slight droop to her nose when she smiles," he said. He added, regarding the mom of four: "when I compare back to photos of her in high school, what I'm seeing is that she still has some width to her nose but the tip looks more narrow and less bulbous than it did back then."
This, he explained, "suggests a possible rhinoplasty surgery in which the nasal tip was subtly narrowed to not deviate too far off from her original nose, but to give her some additional contouring and to take some attention away from her nose and for the focus to be on other features of her face."
Linkov also speculated that Lively might have had lip filler treatments in 2008 when she was 21 years old. "I also see some signs of potential lip filler in Blake's upper lip," he said. He went on: "you can see some potential migration into the cutaneous portion of her upper lip, so that's the area just above the vermilion border."
He then pulled up a photo of Lively in 2009 at the age of 22. "When I look at her eyes, something has changed. It looks like the super tarsal crease is now more visible now." This, Linkov stressed, "doesn't typically happen on its own, so it does suggest a possible upper blepharoplasty surgery." He explained that there was "more of an opening to the upper eyelid, and I think that might be from an upper blepharoplasty."
Another important year that Linkov highlighted while examining Lively's features was 2013, when she was 26. "At the age of 26 I'm seeing some signs of nasal tip BOS on the left." He said that "these can arise soon after a rhinoplasty or they can arise later on, like years later." The doctor pointed out that this is usually "because of some scarring, and scar contracture that leads to these different types of prominences on the tip of the nose."
One year later in 2014, the surgeon noted he saw "signs of a potential revision rhinoplasty." The tip of her nose appeared "smaller and that she had a more pronounced super tip break," he noted. "The bridge of the nose appears more narrow, and that to me suggests that she underwent a potential second rhinoplasty to further reduce the size of the tip of the nose and narrow the nose overall."
Linkov stressed: "you can see how different her nose looks now." In 2015, when the Age of Adaline alum was 28, the doctor said that he saw "some signs of the start of Botox treatments." When analyzing a photo of the Sisterhood of the Traveling Pants star from the following year, the surgeon said that any changes made to Lively's appearance were "gradual and non-dramatic, and this leads to a more natural look that is less noticeable.
He also mentioned that Lively's natural beauty significantly contributes to her overall glow and radiance. "In 2019 at the age of 32 you can see how beautiful Blake's skin is and this really makes a big difference in her overall appearance," he said. Linkov added: "there's almost this glowing shine that she has and that stems from good skin care."
He compared a photo of Lively from 2021, when she was 34, to one from her teenage years at prom. He observed that in the more recent image, she had fuller eyebrows and her eyes seemed more open, possibly due to a rumored upper blepharoplasty. He also pointed out that her nose appeared slimmer, which drew more attention to her eyes. The thicker eyebrows further highlighted this area.
Additionally, her upper lip seemed fuller, adding a sense of harmony and balance to her face and reducing the visibility of her gums. Around that same time, Linkov also spotted signs of "slight forehead Botox" but noted that it wasn't "too strong." All of these "small changes really add up," he concluded, guessing that the Betty Buzz mogul and style icon could have spent $125,000 on the treatments he talked about.
