The "Before He Cheats" songstress took to Instagram to share pictures of her new tattoo, which she got as a group bonding activity with her mother and two sisters. 3 of the Underwood gals got black heart outlines on their wrists, while one got the same design on the top of their foot. Carrie's caption on the photo carousel read, "When your 74-year-old mother asks you and your sisters to get matching tattoos with her in Vegas, the only questions are, 'what?' and 'where?' I never would’ve thought I’d see the day that Mama Carole would be gettin' some ink!" followed by a crying laughing emoji, a heart emoji, and the hashtag #WhatHappensInVegas.
It seems Carrie was the Underwood that got the tat on her foot, likely due to concerns about having to cover it up for photo shoots if it were in a more visible location. The country superstar's picture carousel featured images of a tattoo artist inking the design on herself and "Mama Carole", along with group photos of all the Underwood gals together. Carrie's family was likely in town to see her perform at Resorts World, where other big-name artists like Kevin Hart and Katy Perry have had residencies as well.

Carrie's cute new heart tattoo wasn't the biggest attention grabber of her post, however. Many followers ended up debating if the musician had had plastic surgery, and if so, implored her to stop. "Stop carving on your face. You are so beautiful just the way you are," pleaded one of the top-liked comments. Another reply that garnered 210 likes wrote, "Carrie Underwood’s face is starting to lot not look like #carrieunderwood , don’t ruin your beautiful face with plastic surgery please," along with a prayer hands emoji.
This wasn't the first time that speculation ran rampant about Carrie's possible plastic surgery—in fact, those questions have arisen since at least 2017, when the singer suffered a freak falling accident that left her with 40 stitches in her face and an altered appearance.

Carrie Underwood has denied getting plastic surgery after the 2017 accident, calling speculation about her appearance "sad" in an interview with Redbook the following year. “I’m on some magazine every other week for something crazy. It’s a little sad because the truth is just as interesting. I wish I’d gotten some awesome plastic surgery to make this [scar] look better,” the American Idol alum confessed.
She has also stated that she doesn't "worry too much" about the rumors of her possible plastic surgery—but fans think she isn't being up-front about her possible procedures. One wrote in her Instagram comments, "She did not look like this after her accident, that was years ago, this is plastic surgery to stay young." Some commenters, however, did argue that the stitching and scarring from the accident is what has altered her appearance, and those speculating on if it was plastic surgery are being "rude". "Actually she had surgery from her injury she had years ago! She severely cut her lip and nose, after a fall down some stairs," explained a frustrated fan. "You should read her book! It was traumatizing for her!"
Carrie has not responded to comments asking if she has had plastic surgery, or advising her to stop.
